THE EVIDENCE BOUNDARY
Retatrutide and B1: evidence, hypotheses and unknowns
The Rethiamine programme is built on a strict separation: published, verifiable findings on one side; Panacea Bio Chem’s working hypotheses on the other. This page is the definitive statement of that boundary.
What is established
- Retatrutide is an investigational once-weekly triple receptor agonist.
- Thiamine is required for cellular energy metabolism.
- Humans maintain limited thiamine reserves.
- Severe restriction of food intake and prolonged vomiting can contribute to thiamine deficiency.
- Nutritional monitoring is relevant during substantial pharmacologically driven weight loss.
What Panacea is investigating
- Physical integrity and reproducibility of the dual-layer Peptourbillon™
- Retatrutide stability in the lower lyophilised layer
- Thiamine stability in the upper lyophilised layer
- Interlayer migration during freezing, lyophilisation and storage
- Stability during manufacture and storage
- Rapid and complete reconstitution with 0.5 mL P-EARLs™
- Reconciliation of pH-, pI-, ionic-strength- and solubility-related conflicts
- Post-reconstitution compatibility
- Subcutaneous local tolerance
- Whether a weekly thiamine component can meaningfully support thiamine status
- Whether the formulation affects retatrutide pharmacokinetics or activity
How to read the established column
Each established statement is supported by the verified reference library: retatrutide’s pharmacology by the NEJM and Lancet trials; thiamine’s metabolic role by current biochemical reviews; the deficiency mechanism by established clinical nutrition science; and the monitoring point by a 2026 Nutrients review of nutritional management during anti-obesity medication. None of these sources studies retatrutide combined with thiamine — the combination itself lives entirely in the right-hand column.
Explicitly not established
- That retatrutide directly consumes or chemically depletes thiamine.
- That any individual receiving retatrutide requires thiamine supplementation.
- That a weekly co-formulated thiamine component improves any clinical or nutritional outcome.
- That the dual-layer Peptourbillon™ preserves separation across the full shelf life — this is under analytical investigation.
- That all pH and pI conflicts have been eliminated — reconciliation is an engineering objective under evaluation.
Why the boundary matters
Potent weight-reduction pharmacology is advancing faster than the nutritional science around it. A formulation programme that blurred the line between established concern and investigational response would overstate both. Rethiamine states the concern precisely — retatrutide-associated nutritional vulnerability — and states equally precisely that the response is a research formulation under development, subject to the analytical and pharmaceutical programme before any clinical question can even be asked.
